Happy you found the cause and solved your dilemma. Something like this happened in my game recently. An NPC died and a crowd of Sims gathered to mourn. Unfortunately this was enough to block Grim from getting close enough to do his job. In my case I had my Sim get close enough to call over /chat with a couple of the mourners. That left enough room for Grim do do the reaping.
