SinfulRoyal I've had this happen randomly on and off for at least the last 2 years. It has happened with multiple sims, but only one at the time and it may last for a few minutes to hours, and it may happen across play sessions. It happens with a sim wearing game attire or CC. I haven't had it happen with the active household, only with NPCs. I see it mostly when I travel away from home to another zone.
To get it to go away, you can wait it out (which is what I do now) or you can go to CAS and change the outfit. If it shows up when you arrive to a travel destination, it will usually clear up if you travel out and then back to that location.
