Sim lost weight + her appearance changed

As the title says, my sim lost weight and her entire appearance changed. her hair colour, skin colour, makeup, facial features.. everything. I managed to get her actual sim looking how she did before - but the picture with moodlets stayed the same, wrong sim. its lowkey driving me crazy and i have tried restarting the game, and it only solved the moodlet picture, not the minimised one. she is the sim called Adrianna that im hovering over. 

does anyone know what may have caused it and whether theres a fix? or if i should just deal with it being wrong...
