Check family inventory. It may have been sent there for some reason - but all my sim babies born in the hospital always get one. A few times I noticed I found them in my families inventory when they are not in their personal inventory when the sims get back home. One thing to note - I have no mods or CC - so nothing to interfer with the normal workings of the game - might be why my sims always get their BC's. It looks much like diplomas sim in games with Uni get.
Never had one. Not even once. I think the majority of my sim babies are born at a hospital
