@Lazzial The Sims 4 has no blood or nudity or p*rnographic and explicit imagery. It only features sexual themes. M ratings are reserved for much more explicit games. The Sims 4 is nothing. It's even rated G in some European countries.
What you're describing would be rated A+ adults only. The M rating specifically says "sexual content". Just because it's not explicit in the game doesn't mean it's not there because we all know it is.
