The only affair storyline i did was with my sim self's late son Tommy. He married Miki Akiyama right out of high school in 1980. By 1985 Tommy had a career change from coffee shop owner to developing a career in politics. His political advisor Amira grew quite fond of Tommy and Tommy grew quite fond of her. They had a few romantic interludes while at work and at one point was going to leave Miki for Amira.
Tommy got stressed from hiding his affair with Amira that he decided to end things with her and confess all to Miki. Once Tommy had confessed all to Miki, Tommy dropped dead right afterwards:

Miki pleaded to grim to spare Tommy but on this day, Grim wasn't in a sparing mood:
