You feed the Cow people, usually it spits out the people first though rather than eat them, but the chances are still there!
And your sims will feel "Drained" which then you can Milk the cow.
If a Sim was Spit out you can milk out, that sims "Emotion", and your sim will just get a buff.
If a Sim was killed by the Cow, your Sim can milk out someone's life essence, which will extend the life of your Sim.
