Revolt: random even when things are great?
Does the game 'trigger' a revolt as a random event even if things are doing great with every tenant?

I don't think anyone has clarified this. If you're getting tenant revolts even with high maintenance levels and regularly being kind to individual tenants, and you don't have mods (though I don't know of any that could touch this system), you might want to make a bug report at AHQ.
