Sim prisoners - tips needed to prevent escape

Hey I know this string is a little old, but did anyone ever figure it out? I am having the same issue. I started out thinking I would try the asylum challenge, made 8 sims in the same household, built a secure (I thought) asylum, but then I decided to turn the sim I control into nurse Ratched and since she's already insane turn her into a serial killer while running the asylum.
I set it up so any door that would lead outside or to her personal space was locked to everyone but her. Worked fabulously at first, but all of a sudden EVERY TIME she leaves the lot, or I switch to playing another household, my lunatics are freely running around the sim world!
Also, same thing with her "victims". I lock them securely in there comfy little cells, but if I dare to leave the lot I return to find them escaped!!
What's any self respecting psycho killer supposed to do? ??
