Yep, it’s this. They don’t get the moodlet anymore by the time they’re a Teen or older, but the thought bubble still randomly comes up for some reason. Pregnant Sims also get the diaper thought bubble sometimes, but it doesn’t seem likely that’s the reason in OP’s case.
Yes, I checked and all four teens have the Happy Toddler I guess they're just reminiscing of the good old days. LOL

I'll have to pay a bit of attention to this - I've noticed my kids getting the diaper thought bubble, and always wondered whether it was a random thing or meant something specific. They always have the happy toddler trait, so that would confirm this theory.
Family-oriented adults also get the diaper bubble, but in that case, I've always figured that they want another baby.
