- funny you should ask now
I didn't really count the dead
but once i mistakenly went to outdoors spa at winter to test it out and it was a whole death fest there with sims freezing to death left and right
i think it was at very least 10 sims before my sim went home
grimmy was busy busy - I think around 10. I wanted to get rid of some of the NPCs while also collecting tombstones for my cemetery. I would add The to my household and then drain their hunger with UI Cheats.

My sim went to work one day and managed to visit a crime scene not knowing the horror that would greet him upon returning to the station. First the police chief, then the desk sergeant, then two other officers in rapid succession it was absolute chaos. He only got bronze for the day, and was sad for days after but it was pretty hilarious. >:)
- This was sims 3. To this day, this only ever happened to me that once and I never knew it could happen in the first place.
The son of my mayor had just graduated high school and I moved him into his first place, which was a tiny home in an up and coming neighborhood. Now when I say tiny, I mean 4x8, carelessly missing a smoke detector and it has a second floor. I threw his birthday party at the tiny house and invited almost the whole town. I see my mistake now. As he went to blow out the candles, everyone gathered around him...every single tile had a sim on it. And as he blew out the candles, he blew himself on fire (that can happen!?). So I couldn't move him through the crowd. The crowd stood there panicking instead of running to safety. I watched as around 25-30 of my townspeople burned to death. It was a very somber game after that. My town went through a depression...like they wouldn't do anything for at least a full sim week after that. It became part of the towns history and I erected a monument and museum. ?
It was an accident, I swear! - My record is five! Once I sent Anita to the gym, it was a beautiful gym I've downloaded from the galery, that had a sort of balconishy yoga room in the second floor, made with glass fence instead of walls... I was new to the game and didn't knew that sims could die because of the blizzard being technically inside of a building... short story, while Anita was doing her workout, I started to see a gathering and other sims mourning the death of a frozen yogi... and then the mourning sims started to die by their turn, because they were all wearing sports clothing... :o
- @Sindocat , I super can imagine it happening... I gave up letting my sims live in high rise buildings in TS3 because of the routing issues!

I believe I recall you sharing that on What Happened In Your Game Today, or some such thread. I was really sorry to read about it.
My advice, exit without saving! - I created a big Frankenhousehold of the max 8 sims, put them all in their winter clothes, then sent them to my pauper's-grave lot in Oasis Springs on a scorching hot summer day. A little bit of "tell them all to cloudgaze then turn autonomy off temporarily" and an hour of game time later, and ol' Grim showed up, no doubt with the same look on his face under the hood that I get at an all-you-can-eat barbecue restaurant in the heart of the American South.
When Grim was done, I was like "don't go anywhere bud, there's more where that came from." I then dumped another 8 lucky contestants into the arena, same method, same result.
And then, for the pièce de résistance, I threw in another 4.
So total of 20 sims culled in one mass death event before I saved and went into Manage Worlds. This was fairly early in my current simworld after I bought this desktop computer; I had to clean house of the more bothersome NPCs.
