While I was never one to enjoy killing off Sims, I do agree that the game has gotten a bit too safe. My first time ever playing a Sims game, back in Sims 1, I had a Sim with zero cooking skills use the stove. He promptly set himself on fire and died. As it was a single Sim household, that was Game Over. Fortunately, I was able to restore from a save point.
(This Sim also didn't realize he had to water and weed the flowers around his lot, and after hiring a gardener he ran out of money, prompting the gardener to literally walk off the lot with one of his chairs as payment. This happened twice. Hilarious.)
In many, many hours of playing Sims 4, I've only had one Sim die of something other than old age so far, and it was a chore to make happen. I had an elder Sim WooHoo with his adult wife three times in a row. I got the "dangerously tired" moodlet after the first time, but had to WooHoo twice more before he finally died. One of the times was even in a rocketship. (Maybe the zero G environment preserved his health?)
