Just like you can go outside in the winter without too much clothes on for a while so can the sims. It is only when you are exposed to the cold for some time that it becomes dangerous. If it was freezing inside your house and you had no way to protect yourself from the cold you would also die. But yes you could have saved her by giving her a cold weather outfit or installing a thermostat from BB.
Sims can die from accidents and it has been like that from the very start. My first accidental death many years ago was a death by hysteria. I learned to keep an eye out for that and calm them down in time.
To get directly to manage world use the load button instead of the continue button.
To be exact, the sim had cold weather clothing and the house is equipped with a thermostat. But as it was summer last time I did play them it was set to cold...
Anyhow I did get a tip to simply go round the issue with Origin not wanting to start offline. The tip to simply terminate the connection to the internet, in this case a WiFi connection. After that the client may think what it wants...I'm offline! 😛
I should have began playing some time without saving just to check but as I did some changes to things I did save anyway. And so far, the game runs good with no losses of lives. The phone rings ever so often but I ignore it every time. Better safe then sorry as I heard of a death of a sim last time I (my sim) did answer it. And I still have 118 sims out of 150 (the maximum). That means that so far no sim have died. Yes, my sims is not effected by weather, it's been turned off (which is boring as I think they won't change outfits depending on weather), but as told: Better safe the sorry.
So, problems solved then...?

If it is not hot it should be set to normal not cold. If you have an AC unit in real life you would not have it on if it was only 20-22C even though it is summer.

And if I play a family in summer when it's hot outside, then stops and the game throws me into the family in winter when it's cold? Then the AC will be set to "Cold".
With all respect (Mr?), my English might not be very good but I consider it to be decent at least. But I almost get the feeling that you want to misinterpret my posts from time to time or focus on something different then me. I don't know why it's so but that's a rabbit hole I don't want to go down.

I'm a rotational player, too, and I know exactly what you mean about the thermostat. You play the family in summer, you set the thermostat to cool because it's summer and hot, you play, you rotate out of the household. The next time you play the household it's winter and they're all complaining that it's "morgue-like."
I use LittleMsSam's Automatic Thermostat mod for this. Otherwise I have to immediately update the thermostat every time I rotate into a household, and that gets old.
Er, as to your other points about Neighborhood Stories, you can disable accidental death, or Neighborhood Stories altogether... I like the feature, but I do understand that not everyone does...

Yeah, the thermostat is tricky. The sim who got turned into ice recently was unattended but before this massive update I did lose a sim just because I did open the lot in winter and it was set to cold. That was my fault and she were also saved in the end. Now, I will play test this updated game for a while but eventually I will turn the weather effects on again. Because I do find it boring that they won't change the clothing. (Maybe they do, but I did not see it happen - they were all in everyday wear).
Yes, I have been disabled Neighborhood stories from the main menu. I'm a "control freak" (or "overprotecting" if you so like) and want to know that my sims are safe and don't do anything stupid. If they do move around to places or other sims then it's OK. I can also live with them interacting with other sims. Everything else is my "ballgame". If anyone enjoys playing with NS on it's their decision. All respect to that.
