Finally found out the issue. The More Randomised Townie Names mod also causes this problem.
I've updated both mods and have no issue with this anymore.I also have this bug ever since Cottage Living came out but I have none of the mods mentioned. I removed and updated my mods but my townies are still born and generated without a last name. Even when my active household sim gives birth, the baby doesn't automatically have a last name, but I have to type it in.

I have this exact same issue. None if the mods above or other name mods, was told to set mccc back to default and did so, 2 new random townies without last names in a fresh test save file :/ I'm waiting for townies to have kids now via NS and see if they get surnames but I doubt it

NS adopted babies get surnames, born babies dont. (Invluding my own thry dont either automatically but adopted do), some randomized townies get surnames and some don't. So my main problem are the born babies - they also got no surnames when using mccc for them so I know that's not related to what I use I'm also getting this, random townies without surnames. I haven't noticed with babies. I did have a name mod but I removed it when I originally saw this thread, however I still have the issue, frustrating!
