Try the following
- Close the game
- Rename Documents\Electronic Arts\The Sims 4 to Documents\Electronic Arts\The Sims 4_Old
- Start the game. All your content will be gone now as the game created a new folder. All your saves/.. is still in the old folder so not to worry
- Start a new save game
- Check if you still have the error
- Report back with your results for further steps on how to get your stuff back.
I've done as suggested here. I don't have any mods or cc. I renamed the folder, then started a brand new game with a married couple. They have all the usual romance options. How do I go back to the game I was playing, with a family I've been playing for several generations, and have the yound married couple be able to romance each other normally, and not just "offer massage"?

Hi @susanshoe2 . Go to your renamed Sims 4 folder, then into Saves in that folder, and copy the files (not the folder!) into the newly created Sims 4 folder. You'll then be able to load your previous save. Hopefully, the folder refresh will have solved the issue.

Thank you 🙂 I renamed the new folder, then un-renamed the old folder, and deleted the new folder to go back to my original. I was still having the same issue, so in the end I installed MCCC, deleted all the relationships except the husband/wife for the newest sim (the game had made her the aunt of her husband because I had her adopt his toddler brother), then just went through re-friending them all again and it seems to have worked.

I have just had this same problem happen to my game. My married couple has 5 children and now all of a sudden, "Offer Massage" is the only option under the romance category. I've renamed my folder, copied the save files to the newly created game, and placed the old save files back into the newly made game. The correct options are still missing. I have cleared my cache and removed any mods. Please advise.....

@GratefulAnner Try a clean folder to see if that resets it: Drag your whole Sims 4 folder from Document > Electronic Arts to your desktop. Launch Sims 4 and reset your Options. Exit and copy your saves files from out of the old Saves folder on your desktop into the new one the game made, and open the game to see if your relationships are back. If that works, you can exit without saving and copy your old Tray files (this is your Library), Screenshots and Videos files. Then reintroduce mods into the game in batches.
