Your dxdiag shows a number of crashes of the graphics driver, so that's the place to start: with a clean uninstall and reinstall of the driver.

You can use the newest driver AMD provides for your processor and graphics chip:

This is necessary, given the errors in your dxdiag, whether or not the driver issue is the reason Sims 4 is crashing. If it doesn't fix the game though, please do remove your Mods folder and test again. And if that doesn't help either, remove Reshade and run a repair (Sims 4 > Properties > Installed Files > Verify integrity of the game files), then try again.
If you find that a specific mod is breaking the game but you very much want its features, list the features you're looking for so that someone can make a suggestion for a mod that actually works properly.
