I'm having the exact same issue! followed these steps with the same problems persisting - my reliability monitor info looks identical to the one provided by OP except for the last section:

Just noticed I also have another one from earlier on stating that it was "TS4_DX9_x64.exe" that stopped working rather than "The SimsTM 4". Not sure if that makes a difference but I might as well share that too as I assume the more info you have the better!!:
