Evolution objective completed but not rewarded/disappeared

I completed the Evolution requirements (played 3 Rush matches), selected Claim instead of Preview, but the Evolution disappeared and did not complete. The player is not upgraded and the Evolution is not available anymore. Please check my account and restore the Evolution progress/reward.
