No Grim/Urn when switching to household with dying Sim

Could you please provide the following information

• Are you using mods/cc in your game ? Please remove them and test without them if you do.
• To be sure, you got a message that they were already supposed to be dead and yet they still were alive when you switched there or was the message that they were about to die ? Or did you load an earlier save ?
• When you saved and reloaded where they still gone ?

No mods. No cc.  I have it turned off in game options.

I did not load an earlier save.  Opened the game as usual.  Playing in 'Xavier household' for awhile. 
Text box pops up that Eve, who lives in another household, is dead and it gave me options to select what happens to her inventory.  I click Add to household inventory.  
I keep playing Xavier household to finish what I was doing there before I switch over to Eve household.  Then a second text box pops up saying that Eve's husband is dead and the same options about inventory. I click Add to household inventory.
Then, I switch to Eve household.
Eve on the floor, dying.  Grim comes inside and does his thing. Family cries. Eve's tombstone is there. 
Eve's husband was not in the house.  He arrives at the house after she dies.
Husband goes to do yoga autonomously.  I'm watching him. Waiting for him to die because of the text box.  AND HE DISAPPEARS.. poof.  No tombstone or urn left. Grim does not show up. Husband is gone from the household.  

I check the daughter's relationships and her father is a ghost.
I look for his tombstone in the house.  I look at the household's inventories.  No tombstone.

Yes.  I just opened the 'Eve household' to double check.  Both Eve and Husband gone and ghosted.

Will check with a CM if EA wants this merged. Putting this on Duplicate until I get some info.

Received one text box for each.   In the past the game has given me the 'death text box' multiple times if I didn't switch households, but not this time.  

Thanks. Hard to tell if it's the same issue, especially if it matches what other have said in that thread.

I'll wait for a CM to make a decision (it's thanksgiving in the US so this can take a bit longer)

Note2Self: My Original title was Abnormal Death/ No Grim

crinrict suggested duplicate below involving one sim who "died" multiple times resulting in  "...the grim reaper keeps reaping the sim resulting in multiple urns."   None of this happened in my game.  Not an accurate equivalence.
