I have the same issue, then started giving regular vet treats and that seems to be working. No toilet brush, but the dog is obsessed with the bathroom even if I moved the location of the room on the lot lol. Now, the only thing really is when I send him to explore he gets sprayed by skunk pretty often, but no illnesses for a while after frequent vet treats (about every 3 sim days or so)!
