Brand new vanilla The Sims 4 folder and Save. New Sim. New pet.
Went to the unowned Vet Clinic that comes with Cats & Dogs. One Vet showed up. It took all day - from 9am to 2am - for my pet to be treated.
Vet seemed to have the Cringe trait and spent all of his time preforming Cringe activities. Whether this is significant I don't know. Might try deleting him and seeing if another generated Sim preforms more efficiently. However, we have no control over the Sim Vets that show up at unowned Vet Clinics nor at owned Vet Clinics if we haven't hired any Vets. Traits like the Cringe trait shouldn't overwhelm an NPC's Role.
At any rate: It should not take all day to have a pet treated.
