Same thing happened to me, even with great relationships, A's in school, all needs in high green. Never hired a butler or nanny of any kind at any point in the save. In the relationships with her family she still showed up as alive and "homeless" in MCCC.
Only way I've been able to get her back is by moving her back to my active household through MCCC, which then shows her icon greyed out like when they go to a rabbithole activity or something like that, and then summoning her to lot by MCCCs mailbox menu. That made her playable again. Haven't unpaused the game since I did so I have no idea if it'll happen again though :(
I also noticed when I got her back her grades were lowered to B's and her afterschool activity (cc) was gone
