This happened to me for the first time today. Worth mentioning, I JUST bought and installed Life and Death. I was playing my household with 2 teens, 2 children and a toddler, all of them were getting notifications about being neglected for having bad grades (despite them all being A students). Went to a funeral event and immediately after it ended one of the child sims "died from neglect" and disappeared from the household and the other sims' relationships panel. I rage quit without saving lol.
