This is happening to me also. I thought it had something to do with Adventure Awaits but I see that has nothing to do with it. I get a message that my child is being taken away due to neglect first, then immediately after i get the big Grim message that the child has DIED!! I do use Mods but they are all updated!!! This seems to be a 1 YEAR OLD BUG that's still has not been fixed!!!!!
