This is a veeeery broken feature that EA keeps adding to packs. I keep having this exact same problem with NPCs from both Enchanted by Nature and from Life and Death and they are packs from 2024/2025. For EbN it seems that once the original NPC gets an ailment they start bugging (it happened in my very first playthru of the pack) but I could still complete the fable quests. For L&D I really don't know what it may be causing the issue but the game doesn't let me complete the tarot quests I get.
