Another quite confusing thing about ailments is that they can be replaced (and thereby prolonged) by other random ailments (or the same one again).
One of my Sim caught one after he talked a little bit to his infected son. Then he got very imbalanced in an instant https://forums.ea.com/discussions/the-sims-4-feedback-en/ebn-balance-system-reacts-way-to-extreme/12335024 and his first ailment changed to another one.
