PC user, tried deleting mods but the problem remained. I had a Sim in another household die autonomously in an accident. This was a household I have played previously but was not playing at the time of death. I went to the house, but no urn or gravestone generated, and it was not in the inventory of the one other member of that family or in the household inventory. But the Sim is gone, not a ghost. Any idea where I can find it?

at which point did you remove your mods/cc ? Did you load a backup and try again with mods removed or just removed them ?

I had saved the game after the death but not after looking for the urn, so I exited without saving, got rid of the mod, and reloaded the game. The urn still did not appear anywhere. I went ahead and reloaded the mod since removing it didn't fix the problem. Another NPC that I had previously played died the next Sims day, and their urn showed up exactly where it was supposed to, so I don't know why this one didn't. But it messed up my plan for my cemetery lol

It's a curious issue and I'm at loss what the game is even supposed to do and why it happens for some and not for others.
Does the neighborhood you currently play in play any role ?

Doesn't seem like it. I'll let you know if it happens again and try to pay more attention to the details.
