I merged your post with the existing thread for this.
Is this for the library (locally) or the gallery (online) for you ? If it's the library, could you try the suggestion I posted earlier ?
(Library) The same if happening to me, started today when I was saving members of a eight member household to individual households crinrict
For context: I noticed that MCCC was importing characters but without their traits and occult forms (if any), I personally don't like that so I decided to take my saved townies households and manually save each member individually so I could replace them properly during my gameplay
I saved each member of "Townies - A" with no issues but when I was in the 7th member of "Townies B" it began to duplicate the household and couldn't delete the duplicates without suddenly corrupting all copies; After a few tries I tried to save the 8th member and the same happened with him... I closed the game and cleared the cache and all, but the 7th member kept being duplicated so I gave up and tried to save the 8th member and save dnormally so I thought it was a problem with that sim especifically
Moving on, I went to "Townies - C" and saved the first sim with no issues, but when I decided to save the second member the problem came back as the image shows, then I came to the internet to find if anyone is having the same issue

have you tried this on a clean user folder ? Maybe something about your tray folder is corrupt (or something inside it). You're library will only contain the Maxis ones but this is for testing if it happens again if you save them again on a clean folder.
I have over 700 files inside Trays, I couldn't possible just start everything again (especially how Idk what is what there) but trying to fix by testing with a new empty The Sims 4 file had worked, it was saving with no issue the few families and individuals that I tested
If you're not opposed to third party tools you can check out the Tray Importer. That might help you figure out what's not working in folder.
