I've been experiencing this problem too. Any time two married sims have different last names, the game seems to automatically fix one of their last names at some point. This happened with both played and unplayed families-- Octavia Moon had her last name changed to Bailey to match her husband's.

this is happening to me as well with the same sims. Octavias last name keeps getting changed to bailey! Its super annoying!
