I'm having the same issue. As an infant the didn't really get any milestones but still got the top notch infant trait. As a toddler they got some of the milestones. But they should be having way more by now. I tested it by letting my toddler read a book but i didn't get the "read first book" milestone while having imagination and communication maxed out. The only skill "only" at level is thinking.
