Update: I've confirmed that in my case, removing the infant from the family and re-adding them temporarily fixes the issue. Also, it seems to be triggered by travelling with any household member to another lot. Really inconvenient when there are household members with active careers.
I have found that leaving your sims on thier home lot similarly seems to work. I have been getting milestones just fine. A pain for active careers indeed, but at least it is hopefully a temporary work around.
