I have been having the same issue with infants. They go through the motions of getting a milestone - music plays, sims go to watch but no milestone is recorded. It then goes through the motions again and again with no result. I haven't notice if it happens with toddlers, mainly because it's more noticeable with infants because they can't progress with their movement.
I play on pc with all packs and minimal mods but this issue happens in all saves including new, mod free saves.
