I'm having this same issue. The first family I made after getting the Growing Together pack had their infant manage to get through a few milestones in one irl play session, but when I went back and loaded the game the next irl day, no more milstones were happening. I made a new family in a different save slot and their infant wasn't getting any milestones from the start - just the little circle that precedes. That was a few weeks ago, today I made another new family, and still no milestones for the infant; even simple things that don't require progress/learning, like first bath or visitor.
I don't have any mods or custom content, just a few expansions and game packs.
