I noticed that after traveling with my parent Sim and letting the infant at home and then coming back, indeed the milestones don't trigger correctly anymore, they do show up in the infants action queue but then disappear without a notification and no milestone in the milestone list.
What helped me to work-around:
After I travelled and came back with the parent Sim, I then go to Manage Worlds and then come back, without any changes, I just load back into the same household. After that my infant's milestone trigger again (until the next time leaving the lot).
