Keep having this issue with my infant in current household. All her other milestones happened just fine, but she keeps getting stuck & looping on the "First Word" milestone. Sparkles, parents drop what they're doing to watch, & the little speech bubble indicating the "first word" pops up. But then, no milestone. And the next time the parents socially interact with her, it just repeats. Cheats don't work to add the milestone either.
Edit: Confirmed exiting to Manage Worlds and reloading the household remedied the issue.
