I have a similar issue. None of the tummy time milestones register.
They have achieved some of the other milestones (first bath, first guest etc.), but no amount of tummy times registers those milestones (turn around to back/tummy, learn to sit/stand, learn to creep/crawl).
The animation is there, my infants are glowing, there are firework-sounds, my sims take pictures, but no milestones.
Edit: Found a workaround: Traveling to a different lot (in my case, a park) and doing tummy time, it unlocked the roll over to back milestone, and now I can teach my infant to sit.
