I have this issue if I use gameplay cheats. I am a console player and I noticed that if I load the game and play without cheating needs things work fine, but as soon as I start cheating needs the infant milestones start giving me popups that they unlocked a milestone but the sims panel never updates to show it. Not sure if the two things are related but it seems to be in my experience.
