Both adult sims have milestone moodlets, but have never experienced those milestones. One has the Sting of Rejection (from Recalling a Milestone), the other has Hated and Sad About It and Bladder Blu...

I've noticed a pattern. I stopped getting milestone moodlets when I turned off balance and luck in the settings. As soon as I turned them back on, I got the sting of rejection moodlet again and another moodlet saying my sim's luck was suffering because of emotional imbalance (my sim was very focused, so I don't think that's true). So this seems to have something to do with balance.

You're likely correct. Though I've tested it a few times, and I'm still getting the same result, so if it isn't balance and luck, it's something that having balance and luck on seems to exacerbate. Perhaps it's just making moodlet switching more common, which might increase the chance of an incorrect one. It might just be a weird coincidence.
I'm curious about what the intended triggers are for milestone memories. I might take a look under the hood.I don't even have the fairy pack so don't have balance and luck and it's still happening to my sims.
