Both adult sims have milestone moodlets, but have never experienced those milestones. One has the Sting of Rejection (from Recalling a Milestone), the other has Hated and Sad About It and Bladder Blunder. I've checked both of their Milestone panels, and neither have milestones associated with these moodlets. I have also never seen these moodlets pop-up for either of them until today after updating the game.

I've been noticing over the last few weeks that my sims have been getting moodlets "from milestone reflections" that have text that don't refer to any milestone that sim has achieved. Eg. "Revived" on a sim that has never died, "Sting of Rejection" from a sim with no relevant milestone, "Hated" on a sim with no enemies. This is happening across multiple saves to sims teen-elder.
No mods installed, all updates relevant, all packs owned.
Mostly this is not a big deal, it's just a moodlet, but it's a little weird.

For at least a month or two I've had this. My sims randomly get the pee themself milestone moodlet, so they get embarrassed for no reason since they NEVER peed themselves. That is so annoying and kind of dangerous for the sims since they can die from embarrassment.
I guess EA will take 5 or 6 years to fix it, but let's hope they WILL fix it someday -_-

It's happening in my game in 2 separate saves in different worlds. I have no mods or CC. I get the moodlet about everyone hating my sim and oddly my sim has never had any negative with any other sim. Then I have one who is getting the Badder one, where she gets an Embarrassed moodlet.
This has to do with random weighted loots.
The milestone about embarrassing memories buff is supposed to work like this: Some action (waking up, going for a walk, etc) has a chance of triggering your sim reflecting on milestones (randomWeightedLoot_LifeMilestone_Reflect). When triggered, it runs through tests to see if your sim meets any of the criteria for the milestone being reflected on. In this case, it would check if your sim has had an energy failure and (I think it's supposed to be or) a bladder failure milestone. If the sim has those milestones, then the sim gets the moodlet/buff about remembering the milestone.
The same idea is behind the other issue some have mentioned here about making an off the grid call. "randomWeightedLoot_OffTheGrid_PhoneUse" There are myriad phone interactions that your sim can do that might trigger that loot, and it runs tests to make sure they're appropriate (on the acting sim's traits and the traits of the lot the sim is currently in). As with the first issue, the tests are being skipped, so you can get moodlets about off the grid phone calls no matter where your sim was when the call was made.
It's not clear to me why the tests are being skipped or why some loots are being triggered inappropriately, but I'm sure the devs are working on a fix because loots not working right suggests all sorts of things could be going wrong that aren't as obvious.
On the one hand I kinda like the random milestone moodlets. Adds some friction and frisson. Like my sims have been living an unseen life while I'm not playing. But, they should be age appropriate.
Why would the developers of the game have our sims feel the "Sting of Rejection" or be "Hated and Sad About It" if that sim has no cause for this? It doesn't make the game more challenging, it makes the game less fun. After all, we play to have fun.
I've had the rejection moodlet from a negative milestone on several sims over the years, but I just realised yesterday what might be the reason for it.
I played a sim that got a wish to flirt with another sim, of course another sim got involved in the conversation just as my sim started to flirt, so it because a negative action/ moodlet for it. Basically he got rejected and it got awkward.
A sim day or two later he got the remembering rejection moodlet, and I checked his milestones but the few he has were all positive. Then I realised the moodlet must be from the flirt rejection he had, I had only played him for a short while until then and this was one of his first interactions with other sims.
I don't even know if there is a moodlet for rejections, I think not, so it's probably just that the moodlet is from rejection but there is no corresponding milestone that we can see (but there might be one in the background), or the text on the moodlet is wrong when it refers to a milestone.
My Caleb has the Bladder Blunder one.... Caleb Vatore... the VAMPIRE with NO BLADDER NEEDS! -facepalm-
