This is a 10-year-old bug that keeps popping back every now and then and it's only for a child alien. I don't use mods.

I can confirm I have this in my game. It appears to be a single, isolated face of geometry. I was never able to identify what specifically it belongs to but I can confirm that it only happens to alien children. It goes away when they are in their human disguise.
