Yes, I've seen this in my game since the Enchanted by Nature update just before the expansion released.
First started noticing it when I was playing Discover University and the mascots would not have the head piece on. And then the opposite happening with other students walking around with the mascot head on their other outfits, which was reported as Situational and NPC outfits generating with random hats - except that they aren't random hats, they're leftovers from a previous situation outfit that aren't being removed.
