I'm also having this issue - normal sim who witnessed his mother's death as a teen, now a young adult - except the Discuss Fears option DOES show up...for every sim EXCEPT ghosts and elders. I can discuss my fears with randos, my sim's young adult husband, his adult mother in law, and even Grim (still no option to give Grim a death flower though), but I've befriended ghosts and elders and the option does not appear anywhere for them. I'm not sure if it's relevant but this is also the only fear he's ever had, so no conflicts there. I'll probably just buy the potion at this point...
