Every game day, my sim gets a notification that the same sim (her son) has died. He does not show as dead on the family tree, but she still gets the mourning moodlet every time and it starts the grieving process over. This has happened the past six in-game days.
I had the same thing going on - A Sim had married into my legacy family and her NPC sister died every morning for a week, yet she appeared as alive in the Sim's family tree. I grabbed her urn and placed it on the family lot, then invited the very much alive sister over on Christmas Eve, whereupon the two sisters argued for the whole of the wee hours until the sister died of rage some time around 6.30am after first becoming enraged and then stuck in the bathroom (the door seems to have 'mysteriously' become locked *nudgenudgewinkwink* as were the house exits). Problem solved. But I do hope the glitch gets remedied.
I didn't know NPCs were affected, too! That's crazy!😯
