I've had this too. PC, all packs except Grunge kit, Goth kit and Bust the Dust kit. No mods. Sitting at level 7 of the Chef career (with the Chef Aspiration being worked on.) I switched her to the Appliance Wiz aspiration, but it still happened. Each time she had definitely done her daily tasks, was in a great mood when she went to work, the meter said she should be promoted, but she got a raise instead (and the meter dropped to the midrange.) Her husband is in the art career and the same thing is happening to him. (He has the tech aspiration, as he finished the painting one.)
It also seems to be related to their work in other ways. When they get home, they usually have the tasks reset to say they need to be done, but they say they're completed. I have them do what usually is needed and in the process it does seem to recognize the tasks are being done as they do get the 25%, 50% and completed that they normally do.
