I had same issue but was able to get it to work. What I saw was during promotions levels were being skipped and the career level number was out of sync with the actual level they were at. For example, with Naturopath career, Apothe-Curist Branch, the text said I was at Master Apothecary (the top tier), but the level number showed 9 (not 10). Once it was out of sync I only saw raises and no promotions.
To fix it, I had to remove the Entrepreneurial Trait from my sim and then restart the career (quit and choose it again. The problem has something to do with how that trait effects promotions. My sim still skipped levels, but the number stayed in sync and I was able to promote successfully to the top tier with tier number matching correctly.
To remove the trait I used the Mod Command Center (MCCC) mod. There might be other ways of doing that but for me, using the mod was the easiest way, I already had it installed. Too soon to say if that always works or if just a fluke, but I wanted to share in case it helps others past this.
