I did some testing, because I have the same issue in my game and discovered, that it was mc command center for me. One sim was pregnant before I installed the pack and I opened the game with mc command installed. The babies were (twins) were born with the secret child trait (in the picture I aged up one of them to see if the secret child trait stays and it does). The father recognised them as stepchildren AND secret children.
The second time around I generated a new pregnancy with mccc and the child (once again) was born as a secret child. However, if the pregnancy happens via woohoo, the pregnancy is recognised as "legitimate" and the baby comes out as daughter/son.
And as the last step I opened my older save file I backuped before I installed the pack and the twins were "legitimate" again.
So I am sure it's a mod bug.

For me, even without any mods at all, the problem occurs.

My game has never had any mods or CC installed at any point, and I am still having this issue. So while it’s possible that mods are having an impact on this in your game, they are not the sole cause.
