I noticed I couldn't add distant relations when I tried to have Cordelia invite the Laurent family to join the Thebe dynasty.
Arnessa Thebe, Hector, Greta & Jace Laurent didn't even show up in the menu as sims she'd need to befriend first - they just weren't there. They show as distant relatives in the family tree, but not in the relationship tab or profile.
Cordelia WAS friends with them, so not quite OP's issue, but this fits the report title.
I have this problem as well. I have tried with mods & without. Im also on PC.
In theory I should be able to add them. The dynasty head is a few generations above the ones I'm trying to add as they are 2 different branches of the same tree.I have this issue as well. My family tree is huge, but distant relatives don't even show up, and others I can't even add even though they show up.
