I also have this problem. My Sim was crowned Grand Duchess (level 9) by Thiago Abrantes. Then she bought a retal in Strangerville, and got demoted to Duchess (level 8). She was promoted once again to level 9, but title "Duchess" appeared again. She sold the retail. Then she pulled Simcalibur from the stone and promoted herself to Queen (level 10), but at some point her title switched to "Duchess" once again.
