I've had this off and on as well as swords disappearing. I'm on console.
I tried to do a sword tournament and couldn’t start the second round. No matter how many times I tried via the gazebo, the rug, or the opponent it always immediately cancelled. I even tried resetting my sim, the opponent (who was chatting), and a couple sims dueling nearby (that my sim kept trying to go watch), but they could not start their duel.
Edit: Fully closing out both Sims and the EA App and then loading back into the game at a later date did finally fix it (sort of). Still had to reset the opponent again who this time was standing frozen in the tournament area, but after resetting him once and queuing the "start next round" interaction again, it worked.
I had already done the optional practice swordsmanship interaction previously and it did not help at the time. I did not do it again when I reentered the game.

Had the same issue, had to cancel the entire event and my sim got no placement and sad and embarrassed moodlet, tried to start a new tournament and then i couldnt duel anyone, the interaction just got deleted immediately

Same here. I had my sim enlist in tournaments. On the second tournament, both sims would not want to proceed and action queued would immediately disappear.
Also, I have notice a disproportionned amount of female contender compare to males... I wish we could have choosen to compete in one of the two gender category or mixed...

I had a similar problem with starting the third round, -however- I had skipped practicing swordsmanship in the event between second and third round, just wanted it overwith. As soon as I went and practiced swordsmanship and got the positive moodlet from it, initiating the third duel round worked just fine. I'm sure there might be other problems and other solutions out there, but this one worked for me. Resetting sims and objects didn't help, quitting and loading in again didn't help.
Nutshell: if you have started a new round of the tournament and 'start duel' doesn't do anything, go practice swordsmanship on a dummy (full action so you get the positive moodlet) and try again.

I found that the sim I was supposed to duel was "fatigued", so I used cheats to remove that moodlet and then the "Start duel" interaction worked. Not sure if this is always the case, but it's the only thing that has actually worked for me so far.

This issue has only happened with me when my adversary is an elder. They'll workout until they get fatigued (very uncomfortable) and then whenever I try to start the duel, it'll pop up a split second before cancelling/disappearing. It's honestly annoying as I have to wait for it to disappear (no mods).
