It might be because of a ghost(s) nearby... I've noticed my ghost (died by freezing) occasionally causes other sims to get really cold even if they are in winter clothes or heated indoors. So there might be a ghost near your sim that died by overheating (or maybe fire) that are causing your sim to overheat. Possibly.
It could be so but I’m not sure. The second time it happened while the already dead sister was around but my second sim had swimswear on and the weather was rainy and chilly, so..idk
Swimwear is not Hot Weather clothing, so it won't protect you from overheating. It's intended for moderate temperatures, just like all the other basegame outfit categories. However, that shouldn't be a problem if the weather was chilly.
